and conditions of the SmartValue plan. You are not required to see a
primary care physician or to get a referral to see a specialist.
And now you have more choices than ever. You may select from two
options: the SmartValue Classic plan, or the SmartValue Plus plan, which
also includes Part D prescription drug coverage.
Monthly plan premiums range from $5 to $73 depending on which plan
you select and your county of residence. (You must continue to pay your
Medicare Part B premiums no matter which plan you choose.)
